Intervencionismo coronario ambulatorio por vía braquial en el CIMEQ

Abstract

Introducción. La cardiopatía isquémica es la primera causa de morbi -mortalidad en Cuba y la mayoría de los países. El tratamiento más extendido es el intervencionismo coronario. Estos han requerido hospitalización durante 48 horas como mínimo.
Métodos: Estudio longitudinal, prospectivo, entre 2004 y 2008. Se incluyeron 152 pacientes (160 lesiones) con implante de stent a todos, luego de 4 horas, alta para su hogar. Evaluados por consulta entre 1 y 4 años. Resultados: Predominó el sexo masculino y la angina estable. La edad media fue de 57,7 años. El tabaquismo fue el factor de riesgo más frecuente. El clopidogrel (300mg) fue suministrado urante o después del intervencionismo en 81,6%. La arteria descendente anterior fue la más afectada. Predominaron las lesiones B2. Tiempo medio del procedimiento 35,4 + 7,7 minutos, éste fue menor con la técnica de stent directo. Se encontró diámetro de referencia pre-proceder 2,9 + 0.4 mm, post-proceder 3,1 + 0,4 mm y longitud de lesión 15, 4 + 4,3 mm. No ocurrieron complicaciones mayores hasta 30 días. Complicaciones vasculares menores 2,6%. La supervivencia libre de eventos fue 95%. Se presentó una muerte por accidente cerebrovascular hemorrágico al año. La reestenosis angiográfica fue 4,4%, con patrón difuso, asociada a oclusión total.
Conclusiones: El intervencionismo coronario ambulatorio po r la vía braquial en pacientes seleccionados con implantación de stent, es seguro y eficaz.
