Gammapatía monoclonal: mieloma múltiple IgD. Reporte de un caso

Abstract

Las gammapatías monoclonales constituyen un grupo de trastornos, que se caracterizan por la proliferación anormal de un clon de células plasmáticas que son capaces de producir una paraproteína monoclonal. El mieloma múltiple es un prototipo de gammapatía monoclonal maligna. Su incidencia anual es de 4 casos por cada 100 000 habitantes. En un estudio de 40 pacientes realizado en el Centro de Investigaciones Médico Quirúrgicas, se detectó una banda monoclonal por el sistema Hydrasys, la mayoría de ellas de novo. La inmunoglobulina más frecuente en este subgrupo es la IgG, aunque algo menor de lo que describe Kyle para la gammapatía monoclonal de significación indeterminada. En cuanto al mieloma múltiple, en la distribución según el tipo de inmunoglobulina, no encontramos ningún caso de IgM, pero sí dos de IgD, el cual según los datos de la literatura es bastante infrecuente. Se presenta a una paciente a la que se le realizó el diagnóstico de un mieloma múltiple IgD, cursando el mismo con una forma de evolución clínica atípica.
