Ética durante la reanimación cardiopulmonar y cerebral del adulto

Abstract

Casi un 50 % de los supervivientes del paro cardiorrespiratorio (PCR) presentan secuelas neurológicas significativas. Por ello, las decisiones en la Reanimación Cardiopulmonar y Cerebral (RCPC) deben basarse en los principios básicos de la ética médica. Nos proponemos identificar los aspectos bioéticos que inciden en el ejercicio de la RCPC, y discutir controversias actuales y las recomendaciones más aceptadas: frente a un PCR se debe iniciar la RCPC, aunque existen criterios válidos para no iniciarla en algunas circunstancias. La decisión de interrumpirla está basada en la consideración de varios factores considerando los principios de autonomía, no maleficencia, beneficencia y justicia. La orden de no reanimar debe estar basada en la decisión autónoma del paciente expresada fehacientemente de forma verbal o por escrito. Se discuten además otros aspectos éticos durante la RCP. La RCPC se debe aplicar si está indicada, omitirse si no está o no ha sido previamente aceptada por el paciente y suspenderse si es inefectiva. “Tener siempre presente, que los participantes somos seres humanos”.
