Escherichia coli en conducto radicular dentario

Abstract

Los microorganismos son la causa más frecuente de patología periapical, ellos constituyen el denominado biofilm que recubre la pared de los conductos radiculares con una pulpa necrótica. A la consulta de Estomatología del Centro de Investigaciones Médico Quirúrgicas acudió remitida una paciente con un tratamiento pulpo-radicular fallido en el 22, con más de 5 años de evolución, a la cual se le trató mediante preparación biomecánica e irrigación con germicidas, luego se verificó la esterilidad del conducto utilizando el thioglicolato como medio de cultivo microbiológico. Se detectaron dos microorganismos, en un primer cultivo solamente el E coli tratado localmente con una pasta que contenía ciprofloxacina en polvo y agua destilada estéril. Al realizar el segundo cultivo se detectó un Streptococous sp (alfa hemolítico) el cuál fue tratado sólo con hipoclorito de sodio al 3%, logrando con esto la esterilidad del conducto en cuestión permitiendo sellar definitivamente el mismo con conos de gutapercha. Se remitió posteriormente al servicio de CMF, donde se le practicó una apicectomía retirando el proceso periapical. El seguimiento radiográfico se realizó a los tres, seis y doce meses de realizada la cirugía constatándose reparación ósea.
