Alteraciones de la temperatura en el trasplante hepático ortotópico

Abstract

Introducción. A pesar de los buenos resultados alcanzados en los últimos años, el trasplante hepático sigue siendo un proceder, en el transcurso del cual, pueden aparecer diferentes complicaciones que amenazan la vida del paciente. Entre estas, las alteraciones de la temperatura han sido tradicionalmente invocadas dentro de las más frecuentes. Por lo que este estudio se realizó con el propósito de evaluar la incidencia de las alteraciones intraoperatorias de la temperatura en un grupo de pacientes con insuficiencia hepática crónica terminal a los que se les realizó un trasplante hepático ortotópico.  Métodos. Se realizó un estudio retrospectivo que abarcó a 50 pacientes adultos a los que se les realizó un trasplante hepático en el período comprendido entre enero del año 2014 y febrero del año 2017 en el Centro de investigaciones Médico Quirúrgicas.  Resultados. Durante la realización del proceder se presentaron alteraciones importantes de la temperatura en todos los pacientes. Las alteraciones que se presentaron durante el proceder fueron ganando en intensidad con el decursar del mismo, culminando muchas de ellas al revascularizarse el injerto, momento a partir del cual comenzó a producirse una recuperación que marchó en paralelo con la restauración de la función hepática. Conclusiones. Aunque durante las tres fases de la operación se presentaron alteraciones de la temperatura, fue durante la fase anhepática y, sobre todo, en la reperfusión del injerto donde estas adquirieron la mayor intensidad, resultando estos momentos los de mayor dificultad en el manejo clínico de estos enfermos.
