Ictus isquémico de repetición secundario a vasculitis aislada del sistema nervioso central. Presentación de un caso

Abstract

Introducción: La vasculitis primaria del sistema nervioso central (VPSNC) es un trastorno infrecuente, afectando a vasos medianos y pequeños del encéfalo y medula espinal. De etiología desconocida, es un reto diagnóstico de formas inhabituales de ictus. Caso clínico: paciente femenina de 42 años de edad sin factores de riesgo vasculares. Presenta déficit neurológico a repetición, con fluctuación clínica, a pesar del tratamiento con aspirina (ASA). Al ingreso se precisa tensión arterial normal, así como otros bio-parámetros. Se realiza Resonancia Magnética Nuclear (RMN) de cráneo evidenciándose imagen hiperintensa en T2 heterogénea a nivel subcortical y peri-silviana del hemisferio cerebral izquierdo con lesión isquémica subaguda, con otras lesiones antiguas en territorio profundo de la arteria cerebral media (ACMi) izquierda. La Angio-RMN mostró múltiples zonas de estenosis a nivel de ACM derecha y territorio cortical de ACMi, planteándose la VPSNC. El déficit neurológico muestra tendencia a la mejoría. La arteriografía cerebral convencional, reafirma los hallazgos encontrados en la Angio-RMN. Se descartan desordenes hematológicos y autoinmunes asociados a fenómenos trombóticos. Se inicia tratamiento con metil-prednisolona, manteniéndose terapia con ASA y continuando ciclofosfamida oral. Al egreso se constata hemiparesia derecha de ligera intensidad.  Conclusión: este caso manifiesta desde la  experiencia en la práctica clínica, se concluye  como un ictus isquémico a repetición con hallazgos angiográficos de VPSNC. Se demuestra su heterogeneidad clínica e imagenológica y difícil diagnóstico, requiriendo alta sospecha clínica. Esto constituye una herramienta para profesionales de la atención terciaria, secundaria y primaria, contribuyendo a la sospecha y el diagnóstico de tan difícil entidad clínica.
