Utilidad del Ca 19-9 y CA-125 en el diagnóstico de las estenosis malignas de vías biliares

Abstract

Introducción. Las estenosis de vías biliares incluyen un amplio espectro de enfermedades benignas o malignas hepatobilio pancreáticas; en ocasiones es complejo determinar la etiología benigna o maligna de estas lesiones por lo cual es necesario utilizar métodos eficaces y poco invasivos como los biomarcadore smoleculares. Métodos. Se realizó un estudio prospectivo de validación diagnóstica en 50 pacientes con diagnóstico de estenosis de la vía biliar por colangiopancreatografía retrógrada endoscópica (CPRE) en el período de un año, las variables principales del estudio fueron tipo de estenosis biliar, estudio citológico de la estenosis biliar y biomarcadores moleculares (CA-19,9; CA-125). Resultados. Predominó el sexo masculino en 60 % de los pacientes. La media de edad fue de 64,7 años. El 80 %de los pacientes presentó estenosis de aspecto  maligno de la vía biliar en la CPRE y el 20 %de aspecto benigna; más del 50 % de las estenosis malignas presentaron marcadores tumorales elevados y citología positiva. Conclusiones: Los marcadores tumorales CA 19-9 y CA-125 son útiles para el diagnóstico de las estenosis malignas de la vía biliar.
