Evaluación de la función ventricular sistólica y diastólica por ecocardiografía en pacientes sépticos

Abstract

Introducción.La sepsis es una de las principales causas de muerte en la Unidad de Terapia Intensiva. La disfunción miocárdica forma parte de la disfunción múltiple de órgano secundaria a sepsis y shock séptico. El estudio ecocardiográfico permite evaluar las alteraciones funcionales y estructurales que se producen a nivel cardiovascular. Métodos. Se realizó un estudio observacional descriptivo, transversal en pacientes con diagnóstico de sepsis y shock séptico en el período comprendido de febrero del año 2017 a mayo del mismo año en el Centro de Investigaciones Médico Quirúrgicas, realizándose ecocardiograma transtorácico, se evaluó la función sistólica y diastólica de los pacientes.Resultados.De los 11 pacientes incluidos en el estudio con una edad media de 66,7 años, la mayoría eran hombres. El 36,3 % falleció en shock séptico. La disfunción diastólica estuvo presente en todos y fue evaluada por ecocardiografía.Conclusiones. La ecocardiografía permite el diagnóstico de la disfunción miocárdica durante la sepsis y el shock séptico. La presencia de disfunción sistólica del ventrículo izquierdo estuvo relacionada con la mortalidad. Dentro de la función diastólica; las menores velocidades de la onda è y el valor del cociente entre E/è más altos fueron predictores de mal pronóstico en la sepsis y shock séptico.
