Neurolisis intradiscal lumbar con azul de metileno como tratamiento del dolor discogénico

Abstract

Introducción. Con el propósito de evaluar la eficacia de la discolisis con azul de metileno, para aliviar el dolor discogénico lumbar, se realizó un ensayo clínico terapéutico en la Clínica del Dolor del Centro de Investigaciones Médico Quirúrgicas, en el periodo comprendido de enero del 2015 a enero del 2018. Métodos. La muestra estuvo constituida por 50 pacientes, que fueron asignados de manera aleatoria a dos grupos de estudio: A-discolisis con azul de metileno; B-discolisis con ozono. Ambos procedimientos fueron realizados con anestesia local y control fluoroscópico. Para evaluar la mejoría clínica, se tuvo en cuenta el grupo que obtuvo un número mayor de pacientes, en los cuales coincidía una evaluación del dolor inferior a 2 y un test de Lattinen igual o inferior a 4; también se evaluaron los eventos adversos ocurridos. Resultados. En el grupo A el número de pacientes que obtuvo mejoría clínica fue de 24, mientras que en el grupo B fue de 15 pacientes (p<0,001). El número de complicaciones fue inferior en el grupo A (p<0,001). Conclusiones. La discolisis con azul de metileno resultó más eficaz para aliviar el dolor discogénico lumbar.
