Colocación de reservorio subcutáneo venoso central permanente

Abstract

Los catéteres reservorios venosos subcutáneos son sistemas implantables que constan de dos componentes: el portal o reservorio y el catéter radiopaco. Estos dispositivos proporcionan un acceso sencillo, seguro y permanente al sistema vascular en la realización de tratamientos endovenosos prolongados y ofrecen la posibilidad de múltiples y frecuentes punciones a largo plazo, así como la extracción de sangre de una manera menos cruenta. La decisión de colocar un acceso venoso central debe realizarse después de considerar el riesgo y beneficio en cada paciente. Para su inserción se canalizó la vena yugular interna, se colocó el reservorio debajo de la piel del tórax, se tunelizó el catéter y se introdujo en la yugular. Se describe la colocación de un reservorio venoso central, en un paciente masculino sometido a tratamiento con quimioterapia, el cual no presentaba accesos venosos periféricos, que fue remitido al servicio de radiología intervencionista del Centro de Investigaciones Médico Quirúrgica. El dispositivo fue insertado sin complicaciones inmediatas, de forma ambulatoria. Esta técnica, sencilla, aunque no exenta de complicaciones, se realizó por el radiólogo intervencionista, lo que beneficia a muchas personas y eleva la calidad de vida de los pacientes.
