Granuloma eosinófilo espinal dorsolumbar operado

Abstract

Se presenta un caso de un niño de catorce años que debutó con un dolor en la espalda, asociado a trastornos de la marcha. Se realizaron estudios simples y contrastados de la columna, demostrándose una fractura de la vertebra dorsal 12 con compromiso medular. El estudio clínico e imagenológico realizado de urgencia, conllevó rayos X simple de columna dorsolumbar anteroposterior y lateral, mielografía y mielo - TAC 64 cortes. Inicialmente se impuso tratamiento con esteroides llevándose al salón de operaciones antes de las 24 horas, con el diagnóstico de compresión medular, donde en conjunto con Ortopedia y Neurocirugía, se le realizó una laminectomía descompresiva a nivel de los espacios D11-12-L1, colpectomía de D-12 y resección del tumor con instrumentación de Luque; posterior a lo cual desaparecieron todos los síntomas iniciales.

Se concluyó que ante un paciente con cuadro clínico de compresión medular sin antecedentes traumáticos, puede pensarse en una lesión osteolítica a causa de un tumor.
