Uso de dipirona – tramadol - diclofenaco vs dipirona - diclofenaco en la analgesia preventiva del dolor postoperatorio

Abstract

Introducción. La efectividad en el manejo del dolor postoperatorio implica un abordaje multimodal y preventivo. De ahí que fue importante evaluar la efectividad analgésica postoperatoria de Dipirona-Tramadol-Diclofenaco frente a Dipirona-Diclofenaco en las cirugías de abdomen inferior. Métodos. Se realizó un estudio descriptivo, longitudinal prospectivo, que incluyó 70 pacientes programados electivamente para cirugía intra-abdominal en el
Servicio de Anestesiología y Reanimación del Hospital Dr. Joaquín Albarrán durante el período comprendido entre septiembre de 2013 a Septiembre de 2015, los que fueron divididos en dos grupos de comparación según modalidad de analgesia preventiva empleada: Tramadol + Diclofenaco/Dipirona (n=35) frente a  Diclofenaco/Dipirona (n=35). Resultados. El empleo de Tramadol +
Diclofenaco/Dipirona mostró una tendencia a mejor calidad analgésica respecto al grupo Diclofenaco/Dipirona (p=0,072). (EVA <4 puntos en 68,6% frente a 48,6% p=0,124). Las náuseas, vómitos y epigastralgia, fueron los principales efectos adversos, con una presentación similar entre ambas modalidades.
Conclusiones. La combinación Tramadol + Diclofenaco/Dipirona mostró una tendencia a mejor calidad analgésica en el postoperatorio de cirugía abdominal inferior, con similar incidencia de efectos adversos respecto al empleo de Diclofenaco/Dipirona.
