Rash cutáneo después de toma de amoxicilina en adolescente con mononucleosis infecciosa

Abstract

La aparición del rash en la mononucleosis infecciosa no está totalmente aclarado que sea por sensibilización al antibiótico. Todo parece indicar que es el propio virus Epstein-Barr el que induce una reacción inmunoalérgica, lo cual produce el desarrollo del exantema maculopapular después de la ingestión de amoxicillina en la infección de mononucleosis infecciosa. Se presenta un paciente diagnosticado con mononucleosis infecciosa, el cual fue tratado previamente con amoxicilina y presentó un rash maculopapular severo. Se le realizó el test de Prick más la prueba intradérmica a la amoxicilina. Se muestra un test en vivo positivo a las penicilinas quedando pendiente el test en vitro de transformación linfocitaria a los dos o tres meses de haber desaparecido el rash. Como está descrito en la bibliografía se concluye que hasta ahora este rash severo al uso de amoxicilina es inducido por la reacción inmunoalérgica del virus Epstein-Barr.
