Asistencia circulatoria mecánica en la preservación de la función renal y esplácnica

Abstract

Se presenta un paciente masculino, de 37 años de edad, de la raza blanca con antecedentes de herida por arma de fuego en región dorsal, de seis meses de evolución, con síntomas de disfagia y dolor torácico posterior. Se realizó una aortografía y se estableció el diagnóstico de pseudoaneurisma de la aorta torácica a nivel de T 10. Por la alta posibilidad de rotura y los síntomas que estaba ocasionando se decide la intervención quirúrgica. Se le realizó como vía de abordaje, toracotomía izquierda posterolateral a nivel del 7mo espacio intercostal, una segunda incisión en la región femoral donde se canularon arteria y vena femoral para que durante el tiempo de clamp aórtico a nivel torácico no existiera isquemia de los órganos abdominales logrando una perfusión abdominal por la máquina de circulación extracorpórea y por encima del clamp con el corazón como bomba. Se colocó un parche de dacrón recubierto con colágeno. La evolución postoperatoria fue favorable.
