Stents liberadores de fármacos versus stents metálico convencional seguimiento a largo plazo

Abstract

Se realizó un estudio longitudinal descriptivo, con el objetivo de evaluar la evolución clínica de los pacientes con stents liberadores de droga en comparación a los convencionales. De los 279 pacientes tratados entre mayo del 2004 y agosto del 2006 se tomaron dos grupos: tratados con stents liberadores de fármacos (37 lesiones) y stents convencionales (46). Se excluyeron los pacientes que no aceptaron participar y los que no se realizó seguimiento evolutivo. En los casos sintomáticos con pruebas positivas, se repitió la coronariografía. Se siguieron los eventos cardiovasculares mayores y las reestenosis. Se utilizaron test Chi2, y de diferencia entre medias, curva de Kaplan-Meier y test de rangos logarítmicos. Se produjo oclusión subaguda e infarto q en el 2,2% de los stent convencionales, infarto no q en 4,7% de los convencionales versus 2,7% en los liberadores. Se documentó reestenosis en el 8,7% del primer grupo y 2,7% en el segundo, un número igual necesitó nuevo intervencionismo. Concluyéndose que los stents farmacoativos reducen la incidencia de eventos cardiovasculares mayores y la reestenosis, no aumentando la trombosis tardía.
