Intervencionismo coronario por disección de la arteria braquial derecha

Abstract

Se realizó un estudio prospectivo, no controlado entre marzo y diciembre del 2004, incluyendo 23 pacientes a quienes se realizó coronariografía ambulatoria y según criterios de inclusión les fueron implantados stents coronarios (25 lesiones) con resultados satisfactorios. Predominó el sexo masculino (73,9%) y la angina estable (52,2%). La edad media fue de 55.1 + 8.9 años. La hipertensión arterial asociada al tabaquismo fueron los factores de riesgo más frecuentes. El clopidogrel (300mg) fue administrado durante o inmediatamente después del intervencionismo en el 80%. La arteria más frecuentemente tratada fue la Descendente Anterior (44%). Predominaron las lesiones B2 (64%). El tiempo medio de procedimiento fue de 48 minutos. Se encontró: diámetro de referencia 2,87 + 0.42 mm; diámetro de la estenosis 71,6% + 15.1; diámetro luminal mínimo 0,97 + 0.64 mm; longitud de la lesión 14,72 + 5.6 mm; diámetro del stent 2,9 + 0.42 mm; longitud del stent 16,8 + 5.44 mm; tiempo de insuflación 36,4 + 7.3 segundos; atmósferas 13,56 + 1.7; ganancia aguda 1,96 + 0.61 mm. Dos pacientes presentaron sangramiento ligero en el sitio de la disección (8.7%) y uno (4.3%) hematoma pequeño. El pulso de la arteria radial final del brazo derecho donde se realizó la disección de la arteria braquial se mantuvo presente en la totalidad de los enfermos y persistió a las 24 horas después del procedimiento en la consulta de seguimiento, por tal motivo concluimos que este método realizado en pacientes seleccionados tratados con stents y buenos resultados angiográficos es seguro y eficaz.
