Clinical-surgical management of renal tumor with vena cava infiltration

Introduction: Renal tumors rank third in urological practice, preceded by prostate and bladder cancer, respectively. However, venous infiltration of these tumors at the level of the vena cava can occur in 4–10% of cases, making the management of these patients more complex from a clinical and surgical perspective.
Objective: To describe the clinical and surgical management of a patient with hypernephroma with infiltration of the infrahepatic inferior vena cava.
Clinical case: A 58-year-old female patient with a personal history of thrombopathy presented to the Urology Department with an abdominal ultrasound performed for mild abdominal pain in the context of arboviral disease. She was
diagnosed with a complex condition in the right kidney, which CT revealed as right kidney hypernephroma with tumor thrombosis in the infrahepatic vena cava. Surgical treatment was decided with right radical nephrectomy with inferior vena cava thrombectomy, after renal embolization. The procedure was successful. The pathology study confirmed a clear cell renal tumor with a vena cava tumor thrombus.
Conclusions: Renal tumors are rare and may be accompanied by infiltration at the level of the vena cava.
