Introduction: Gallbladder torsion is a very rare condition in which the gallbladder twists on its mesentery along the axis of the cystic duct and artery, with complete or incomplete disruption of vascular irrigation.
Objective: To present a rare case of vesicular volvulus.
Clinical case: A 76-year-old white, underweight female patient with a history of hypothyroidism and chronic gastritis presented to the emergency department with clinical, ultrasound, and laboratory findings suggestive of acute cholecystitis. Surgical exploration revealed a twisted (with rotation greater than 180 degrees), distended, and gangrenous gallbladder with hemorrhagic suffusion. A conventional cholecystectomy was performed, with a satisfactory outcome.
Conclusions: Gallbladder volvulus is a condition to consider in the differential diagnosis of acute cholecystitis because delaying intervention can lead to gallbladder necrosis and perforation, increasing morbidity and mortality. Its preoperative diagnosis is very difficult and is generally made during surgery.
