Epidermoid carcinoma in the context of inverted nasal papilloma

Introduction: Inverted papilloma is a rare tumor that has received multiple histological interpretations. It is localized, aggressive, recurrent, and potentially malignant, with some association with squamous cell carcinoma. This tumor becomes malignant, and for this reason, it generally behaves very aggressively, causing extensive destruction and displacement of structures.
Objective: To present the case of a patient with a diagnosis of inverted nasal papilloma with areas of squamous cell carcinoma.
Clinical Case: A 52-year-old white male patient with a history of relative health presented with facial enlargement, including the orbits and nostrils, headache, anosmia, bleeding, and nasal obstruction. He was evaluated by the Otorhinolaryngology Department of the "Abel Santamaría Cuadrado" Hospital in Pinar del Río. A nasal cavity examination revealed a nasal polyp. A biopsy was indicated, and the result was an inverted nasal papilloma with areas of squamous cell carcinoma. The patient was evaluated by the medical oncology department, which suspected the presence of an inverted papilloma in the context of stage 4 squamous cell carcinoma, according to the Krause classification. The clinical manifestations and radiological findings were analyzed.
Conclusions: The recommended treatment for these patients is surgery, using an endoscopic endonasal approach, combined with external approaches (conventional or mini-endoscopic trephination). Radiotherapy and chemotherapy are other treatment options for patients not eligible for surgical treatment.
