Abstract

Introduction: Biliary ileus is an unusual cause of acute surgical abdomen that is caused by the impact of a gallstone on a segment of the gastrointestinal tract, resulting in mechanical intestinal occlusion.

Objective: To present an infrequent case of biliary ileus.

Clinical case: A 76-year-old patient went to the emergency center due to colic-like abdominal pain of three days' duration, vomiting, increased abdominal volume, and constipation. An exploratory laparotomy was performed and an impacted stone was found in the jejunum with necrosis (which required resection with end-to-end anastomosis) and a fibrinoplastic process at the level of the bile duct (which is being followed up in an outpatient clinic).

Conclusions: Biliary ileus is a rare cause of mechanical intestinal occlusion that usually leads to enterotomy with removal of the litho. An unusual case was presented, with vascular involvement of the impacted segment that required resection and anastomosis.
