First evolution as a nursing record in a third-level care service

Introduction: Nursing records document the theoretical and practical knowledge of the care process from the written word of the nursing staff.
Objective: To characterize the first evolution as a nursing record in a third-level care service.
Methods: A descriptive, retrospective study was carried out in the period 2015-2019. The universe was made up of the medical records of patients hospitalized in the international service of the Center for Medical and Surgical Research. A simple random sampling was performed, according to the standards for nursing records and observation. The sample consisted of 60 documents from the first observation of nursing. From the main variable, a content analysis was carried out with an instrument that characterized dimensions of structure, content and writing of the written documentation. Data for the categories assigned to the indicators were coded. They were processed in a Microsoft Excel 2007 spreadsheet and descriptive statistical techniques were used.
Results: 97 % of nursing evolutions were identified with compliance in their structure; however, the characterization of the content was non-compliant from the theoretical perspective and in the correct information of the clinical data; The characterization of the newsroom was favorable in its three indicators.
Conclusions: Difficulties and potentialities were identified in the first evolution as a nursing registry.
