Diagnóstico de la coledocolitiasis y resultados de su terapéutica mediante colangiponcreatografía retrógrada endoscópica

Abstract

Se realizó un estudio descriptivo, prospectivo en 451 pacientes que asistieron a realizarse colangiopancreatografía retrógrada endoscópica al Centro de Investigaciones Médico Quirúrgicas en el periodo de enero de 2004 hasta marzo del 2006. La muestra estuvo constituida por 353 pacientes con sospecha de coledocolitiasis. Para recoger la información se utilizaron los informes de CPRE y se analizaron las variables objeto de estudio con el objetivo de evaluar las posibilidades diagnósticas y la terapéutica mediante CPRE para la litiasis coledociana en nuestro medio. Se detectaron litos en el colédoco en ¼ de los pacientes con indicación de CPRE; fueron más frecuentes en pacientes del sexo femenino y en mayores de 40 años. La principal condición clínica que motivó la CPRE en los pacientes con litiasis del colédoco fue el íctero. La eficacia diagnóstica de la fosfatasa alcalina y el ultrasonido no fue alta. El tratamiento de la coledocolitiasis mediante CPRE mostró buenos resultados y baja morbilidad.
