Test de sangre oculta y colonoscopía en el diagnóstico del cáncer colorrectal

Abstract

Se realizó un estudio descriptivo-prospectivo en 212 pacientes que asistieron a la consulta de Gastroenterología del Hospital CIMEQ en el período comprendido entre enero 2006 y mayo 2007, a los que se les realizó test inmunoquímico de sangre oculta en heces fecales y estudio endoscópico del colon, con el objetivo de determinar el valor de la sangre oculta y la colonoscopía en la detección del cáncer colorrectal. El promedio de edad fue de 60,6 ± 14,0 años, con un predominio del sexo femenino. La principal condición clínica que motivó el estudio fue el cambio del hábito intestinal presente en el 28,3 % de los pacientes. El test de sangre oculta resultó positivo en 76 pacientes (36,0%), 34 (16,0%) tuvieron diagnóstico positivo de cáncer colorrectal, de los cuales el 50 % se localizó en el colon proximal, el 91,12 % de las neoplasias eran adenocarcinomas predominando los moderadamente diferenciados. Al evaluar la eficacia de la sangre oculta en el diagnóstico de las neoplasias se obtuvo una sensibilidad y especificidad de 76,47% y 71,91% respectivamente.
