Oftalmoplejia dolorosa. ¿Síndrome de Tolosa Hunt o síndrome paraneoplásico?

Abstract

La oftalmoplejia dolorosa se caracteriza por dolor orbitario y parálisis oculomotora ipsilateral. El síndrome de Tolosa-Hunt (STH), causado por inflamación inespecífica en el seno cavernoso o fisura orbitaria superior, es una de sus posibles causas. El STH es un diagnóstico de exclusión, por lo que ante un paciente con sospecha de padecer este síndrome se debe realizar los exámenes complementarios pertinentes que incluye entre otros estudios las pruebas de imagen cerebral. En este trabajo se presenta el caso de una paciente de 66 años con este cuadroa quien tras diagnosticarle el síndrome de Tolosa Hunt se le indicó un tratamiento esteroideo. Su respuesta fue satisfactoria a pesar de que 11 meses más tarde falleció de cáncer de pulmón. A partir de esta situación la autora de este trabajo se plantea si realmente se hizo un diagnóstico correcto: ¿Síndrome de Tolosa-Hunt o Síndrome paraneoplásico?
