Embolización de fístulas carótido cavernosas. Experiencia en una serie de pacientes cubanos

Abstract

Introducción. Las fistulas carótidas cavernosas son comunicaciones anómalas de la carótida o de sus ramas con el seno cavernoso. Pueden ser de alto o bajo flujo, y traumáticas o espontáneas. Las manifestaciones clínicas más prominentes son oculares y están relacionadas al flujo retrógrado hacia la vena oftálmica superior. Pueden ser abordadas por vía endovascular, arterial o venosa. Su cierre se produce con coils, con polímeros embolizantes, o con una combinación de estos. Métodos. Fueron tratados por vía endovascular 18 pacientes con fístulas carótido cavernosas entre el año 2010 y el año 2017, en la Unidad de Neurointervencionismo del Centro de Investigaciones Médico Quirúrgicas. De ellos, 12 con fístulas directas y 6 con fístulas indirectas. Todas las fístulas directas fueron tratadas con coils GDC 10 o 18. En 5 de las indirectas se usó la vía arterial con histoacryl, el restante por vía venosa con colis GDC. Resultados. Se logró el cierre total de las fístulas en los 18 casos tratados, en 1 caso se realizó el cierre en 2 sesiones. En 2 pacientes ocurrió un cierre carotídeo no deseado que fue bien tolerado; el cierre fue de manera súbita posterior a colocación de coils 18. Conclusiones. La embolización de fístulas carótido cavernosas es factible, tanto por la vía arterial como por la venosa, logrando la obliteración en el 100 % de los casos. La oclusión carotídea súbita se asoció a la utilización de coils 18.
