Fístula carótido-cavernosa postraumática. Presentación de un caso
Abstract
Se presentó un caso portador de una fístula carótido-cavernosa postraumática. Hombre de treinta y ocho años sufrió fractura de base de cráneo secundaria a un accidente automovilístico con persistencia de cefalea y enrojecimiento del ojo izquierdo, cuadro que empeoró en pocos días hasta hacerse específico de la entidad y bilateral, dado por inyección conjuntival, proptosis, quémosis, diplopia, parálisis de la motilidad ocular y acúfenos continuos. Se diagnosticó clínicamente una fístula carótido-cavernosa lo que se corroboró mediante angiografía cerebral. El paciente tuvo una evolución progresiva hacia la pérdida de la visión y el aumento de los síntomas congestivos orbitarios. Se trató mediante oclusión endovascular con espirales desprendibles de platino, posterior a lo cual evolucionó satisfactoriamente.
