Abstract

El cáncer de esófago es una de las neoplasias asociadas a mayor mortalidad; su mal pronóstico se debe a su diagnóstico tardío. En cuanto a su etiología, los factores más claramente relacionados son el alcohol y el tabaco, la ingestión de ciertos carcinógenos como nitritos, opiáceos fumados y determinadas micotoxinas. Su sintomatología se presenta de manera tardía, con diseminación y compromiso del 60% o más de su circunferencia, el principal motivo de consulta es la disfagia. Para su diagnóstico se utiliza como estudio inicial el esofagograma con contraste y en segunda instancia la ultrasonografía endoscópica para determinar el estadio tumoral. La cirugía es el procedimiento que presenta los mejores resultados curativos, aunque con una mortalidad superior al 20%, por lo que es importante individualizar los factores de riesgo de los pacientes que son candidatos a la misma. Entre las técnicas más utilizadas se encuentran las de Ivor Lewis, Orringer y McKeown.
