Estrés quirúrgico y anestesia

Abstract

El estrés quirúrgico se considera como una respuesta inconsciente al daño tisular expresada por cambios autonómicos, metabólicos y hormonales que siguen a la injuria o al trauma. La respuesta de estrés consiste en un importante aumento de la activación fisiológica, cognitiva y conductual. El trauma quirúrgico produce una serie de estímulos que determinan una reacción del organismo en el proceso de restaurar la homeostasis o contrarrestar la amenaza y que se traducen en una serie de cambios de conducta y cambios fisiológicos, los cuales también pueden ser específicos de acuerdo al tipo de intervención. En los últimos años, se ha venido estudiando el efecto de las diferentes técnicas anestésicas, así como de los medicamentos anestésicos sobre el estrés quirúrgico y se ha llegado a la conclusión de que  los mismos modifican la función inmunitaria al reducir la respuesta de estrés y ejercer un efecto directo sobre las células inmunológicas. El trauma quirúrgico produce una respuesta metabólica, endocrina, hemodinámica e inmunológica que puede durar días o semanas y tanto las técnicas anestésicas (especialmente la anestesia peridural), como los medicamentos utilizados durante la anestesia, pueden disminuir los efectos deletéreos del mismo.
