Amiloidosis primaria de la laringe. Presentación de un caso

Abstract

La amiloidosis es una afección caracterizada por depósitos agregados fibrilares de inmunoglobulinas monoclonales de cadenas ligeras en órganos vitales. Puede ser localizada o generalizada. Se clasifica como primaria o secundaria, de acuerdo a si se asocia o no a otra afección. En cabeza y cuello es una entidad muy rara. La localización más frecuente es en la laringe constituyendo menos del 1% de los tumores benignos. Se presenta un caso de una paciente de 26 años, femenina con antecedentes de disnea de un año de evolución. Al examen físico, con fibrolaringoscopía directa se visualizaron varias lesiones rosáceas de aspecto rugoso que ocupaban toda la subglotis y que histológicamente se correspondió con el diagnóstico de amiloidosis. Se realizaron estudios histológicos para detectar la presencia de depósitos de proteínas en otros órganos que resultaron negativos y por estudios hematológicos se descartaron enfermedades sistémicas, por lo que se confirmó el diagnóstico de amiloidosis primaria localizada de laringe.
