Lethality and risk factors of acute myocardial infarction in a hospital in Havana

Abstract

Acute myocardial infarction poses a challenge to healthcare systems globally. It is among the leading causes of death in Cuba. To characterize its behavior in the study setting, research was conducted to describe the lethality and frequency of the main risk factors in patients treated at a secondary care hospital in Havana. The most relevant results showed a hospital mortality rate of 30.7%, a figure that, although similar to that reported in other national and international studies, prompts reflection on care processes and opportunities for improvement.
The comparative analysis between deceased and surviving patients demonstrated that hypertension, dyslipidemia, diabetes mellitus, smoking, and chronic kidney disease were significantly associated with higher mortality. These results are consistent with international literature, which identifies these factors as strong predictors of poor prognosis.

In light of these results, the need to implement measures as a matter of urgency is evident, among which the following are recommended: 1) Optimizing early diagnosis protocols, ideally with the introduction of highly sensitive biomarkers such as troponins; 2) Strengthening secondary prevention strategies focused on the strict control of hypertension, diabetes mellitus, dyslipidemia and smoking cessation; 3) Conducting prospective studies that allow for a detailed analysis of the causes of the high mortality rate and evaluating the impact of interventions.
