Percheron artery infarction: an atypical vascular disorder

Introduction: Percheron artery infarction is a rare condition affecting the thalamus and midbrain, resulting from the occlusion of a single arterial trunk that supplies both structures. Diagnosis is challenging, as its clinical presentation may be mistaken for other neurological disorders.
Objective: To describe a case of infarction in the Percheron artery.
Clinical case: A 67-years-old male patient with a medical history of essential arterial hypertension and type 2 Diabetes Mellitus presented with disorientation, marked somnolence, and vertical glaze palsy. Magnetic resonance imaging revealed bilateral thalamic infarction.
Conclusions: Percheron artery infarction can be difficult to identify due to its wide clinical spectrum. Early diagnosis is crucial for improving patient prognosis.
