Aneurisma poplíteo izquierdo. Presentación de caso clínico

Resumen

Introducción: Los aneurismas periféricos son infrecuentes, dentro de estos sobresalen los de la arteria poplítea que representan del 70-80 % de los mismos. La cirugía constituye el tratamiento de elección en los portadores de aneurismas sintomáticos, en los asintomáticos mayores de 2 cm o en los que se encuentren trombosados durante la exploración imagenológica.

Objetivo: Presentar un caso infrecuente de aneurisma de la arteria poplítea izquierda.

Caso clínico: Paciente masculino de 67 años que acudió a urgencias por dolor y aumento de volumen a nivel de la fosa poplítea izquierda que se constató al examen físico, de aproximadamente 5 cm de diámetro con latido y expansión. La arteriografía por tomografía axial computarizada del sector aorto-ilio-femoral-poplíteo y distal, demostró la presencia del aneurisma poplíteo izquierdo a nivel de la primera porción, no trombosado. El paciente fue intervenido quirúrgicamente por técnica abierta y abordaje posterior. Se realizó aneurismectomía con injerto de vena safena interna invertida. La evolución fue favorable.

Conclusiones: Los aneurismas poplíteos son poco frecuentes y por lo general requieren de tratamiento quirúrgico. Se presentó un caso inusual, de paciente con aneurisma poplíteo sintomático que requirió aneurismectomía con interposición de injerto de vena safena.
