Modelos Predictivos para la recidiva del cáncer de mama basado en el aprendizaje automático

Resumen

Introducción: El cáncer de mama es la neoplasia maligna más frecuente en mujeres en todo el mundo. La prevalencia y la incidencia del cáncer de mama aumentan cada año; por lo tanto, el diagnóstico temprano, junto con una detección adecuada de la recidiva, es una estrategia importante para mejorar el pronóstico.

Objetivo: Este estudio tuvo como objetivo comparar diferentes algoritmos de aprendizaje automático para seleccionar el mejor modelo predictivo de recurrencia del cáncer de mama.

Método: Se realizó un estudio analítico prospectivo longitudinal donde se desarrollaron varios modelos de aprendizaje automático, que incluyen regresión logística, bosque aleatorio, análisis discriminante lineal y AdaBoost para predecir la recurrencia del cáncer de mama. El área bajo la curva, la precisión, la sensibilidad, la especificidad, el valor predictivo positivo, el valor predictivo negativo y la puntuación F1 se utilizaron para evaluar el rendimiento del modelo pronóstico. Resultado:

Las variables triples negativo, Ki-67, permeación vascular y linfáticas como adenopatías concentran el 78% de la importancia predictiva. Los modelos Bosques aleatorios y AdaBoost muestran el mejor rendimiento global en cuanto a área bajo la curva, sensibilidad, especificidad y F1 score detectando 34 de cada 37 recidivas. Conclusiones: Con base en el rendimiento los modelos muestran excelente estabilidad y generalización, con caídas mínimas de rendimiento corregidas bajo la supervisión clínica.
